We are currently hiring Direct Support Professionals to join our team. Our group homes and apartment style programs are located in the northwest suburbs.

Overview: The Direct Support Professional (DSP) is the heartbeat of our organization. As a DSP you will directly support individuals with intellectual and developmental disabilities.

Responsibilities:

Develop positive relationships with the people being served, their family members, support staff, management team, and consultants.
Prepare meals and household task
Assist with medication administration
Support personal hygiene and grooming, including bathing and toileting

Overview: The Assistant Program Manager will be responsible for direct care shifts as well as administrative tasks.



Responsibilities:

Develop positive relationships with the people being served, their family members, support staff, management team, consultants, and other service providers.
Keeping track of individual’s petty cash, bank runs, assisting folks with paying bills, etc.
Track individual’s medical needs and appointment calendars, scheduling appointments, accompanying individuals to their appointments, ordering medications, ordering medical supplies, etc.
Signing individuals up for activities that they are interested in, keeping the program calendar up to date, communicating with team re: plans, etc.
Purchasing groceries and supplies for the program.

Position Description:

Case Management in disability services is rewarding, fast-paced, and challenging. Hammer strives to provide extensive training and consistent support to our case managers so that they can successfully provide high-quality support to individuals with a variety of needs.


We are growing! Hammer recently expanded our supervisory team so that we can grow our case management department and continue to support more people receiving disability services in the State of Minnesota. We contract our work with Hennepin County and provide CADI and DD Case Management services.


Case Managers provide service coordination to individuals with various disabilities, needs, and backgrounds. Individuals receiving CADI services primarily live with mental health and physical health conditions that impact their daily life while individuals receiving DD services live with developmental disabilities or other conditions that have similar impact on their daily lives such as Autism Spectrum Disorder. Many of the folks we support have a combination of these needs and they all have a variety of skills and preferences that make them unique.


Case Managers are vital to helping people with disabilities and their families find their way through the maze of the social service system while promoting self-determination and a person-centered approach.

Case Managers help to identify goals, needs, and preferences then coordinate with service providers to meet these needs. Case Managers maintain a written plan for everyone, manage payment authorization for service providers, and find resources to meet the unique needs of everyone.

This role requires daily documentation of all work as well as in-person visits to the homes of people you support. Hammer providers Hennepin County Case Management and most clients live within the metro area.


Skills of a successful Case Manager:

Ability to maintain tracking systems for multiple tasks with varying deadlines
Computer skills, including use of Microsoft Office, email, and several online programs
Excellent written and verbal communication, with clients and other professionals
Flexibility to adapt to frequent process changes
Self-motivation to complete tasks independently
Willingness to ask questions
Patience to work with people who are going through difficult circumstances
Qualifications:

Bachelor’s Degree in human services related field, required
Experience in disability-services, preferred
Experience working with mental health, preferred
Reliable vehicle and adequate driving record, this position requires travel to in-person visits at client homes

Hourly rather than Salary: At Hammer, we keep our Case Management positions hourly to encourage a healthy work-life balance. It is important to us that our staff work their hours and then have a cut off to be done at the end of the week. Our staff also have the option to work direct care shifts in Hammer’s group homes or apartments for overtime pay. This unique opportunity allows staff a more hands-on work that many of our staff enjoy complimenting the office work of their case management role.
Remote work: Our case management team has flexible remote work policy. We have office space available in Wayzata and hold occasional in-person staff meetings and fun events. Staff can choose their location of work if they are able to get their tasks done and remain accessible for consultation and supervision. Hammer provides a computer, work cell phone, computer monitor, etc. to ensure a successful work set up whether staff are in the office or remote. Our team heavily relies on Microsoft Teams to connect virtually through chat, video call, and group meetings regardless of location.
Bonuses and Pay Increases: Our department has an incremental pay increase schedule. Case Managers also can earn bonuses monthly based on billable hours. We also have started a Mentorship and Support program where staff can earn bonuses based on additional responsibilities, they take on to support their peers and the department.

Position Description:

Hammer Residences is a nonprofit organization that is mission-driven in our approach. For over 95 years, Hammer has supported individuals with developmental and intellectual disabilities to help them live life to its fullest.


Overview: Responsible for preparing all unit-based billing for Contracted Case Management, Manages Hammer Travel accounts, Custodian accounts, prepares and bills room and board.


Responsibilities:

Work with State and County agencies to ensure billing units are available on service agreements.
Work with Contracted Case Managers to maintain data integrity.
Record, analyze, enter, and classify Travel revenue into the accounting software system.
Work with Travel Department staff to verify accounts and process billing.
Record, analyze, enter, and classify Custodian account transactions into the accounting software system.
Prepare and process private pay billing.
Adheres to HIPAA guidelines/regulations
Participates in educational activities & training opportunities and regular team meetings.


Schedule: Monday- Friday, 40 hours/week, with some flexibility in office and remote. To be discussed upon hire.

Qualifications:

High School diploma, but post high school training strongly preferred
