Position Description:
The Home Visitor conducts visits within a family’s community, focusing on optimizing the health and development of young children. This position works with parents and caregivers to provide support, advocacy and service coordination, using trauma informed and curriculum-based approaches. Visits focus on parent-child interactions, development-centered parenting, family well-being, prenatal care (if applicable), goal setting, and connections to resources. Outcomes include increased parent-child attachment and knowledge of child development, self-sufficiency, positive parenting and family dynamics, and an increase in health, wellness, and safety.
Job Requirements:
Complete the Parents as Teachers Foundational 1, Model Implementation, and Foundational 2 trainings to be certified to provide visits to families.
Conduct 1-2 home visits per month (based on number of family stressors) to assigned caseload to promote positive parenting, child and home safety, knowledge of child development, and school readiness.
Address families’ basic needs, including stability of housing, food, income, and other resources. Provide service coordination and support to families as needed and in conjunction with the Families Together social worker and/or other lines of service such as PSOP. Service will include determining goals with the family, locating/referring to appropriate resources and helping the family connect with and learn to use the needed services.
Use the PAT curriculum as required by program guidelines, conducting home visits in full fidelity with PAT requirements.
Maintain records and reports as required by PAT, LSSMN, and the Minnesota Department of Health, adhering to agency policies and administrative directives.
Regularly participate in reflective practice case review with program coordinator.
Participate in program, agency, and community meetings as appropriate.
Conduct regular outreach activities to build community partnerships and attract new families in need of PAT services.
Occasionally provide transportation for families to program events.
Perform other duties as assigned by program coordinator.
Position Requirements:
Minimum AA degree in Early Childhood Education, Social Work, Education, Human Services or related field, and 2 years of experience working with children and/or families in diverse communities.
Bachelor’s degree preferred
Minimum of two years’ experience working with low-income families with children ages birth through five in their homes.
Bilingual preferred (Karen, Hmong, Somali or Spanish)
Demonstrated ability to understand and work well with families of young children challenged by racial, economic, and social disparities.
Ability to coordinate with other service providers such as social workers, preschools, therapists, and physicians.
Ability to communicate in English verbally and in writing with a variety of people.
Maintain a valid driver’s license, current car insurance and use of personal vehicle for agency business.
Knowledge and experience in case management and client advocacy preferred.
Some evening and weekend hours required due to family schedules and outreach activities.

As a Positive Support Specialist, you will work with high functioning individuals, who have severe and persistent mental illness (SPMI) and challenging behaviors, to become more independent. As a Positive Support Specialist, you will:
Assist individuals to be more independent in performing essential life skills such as meal preparation, personal hygiene, and navigating the community
Advocate for the people supported in all aspects of their life in areas such as rights, medical care and in the community
Participate with the individuals in a variety of events and activities
Complete necessary documentation and record keeping every shift
Manage crisis situations safely and effectively. This can include potential use of manual restraints. Candidates will receive training on manual restraints in conjunction with the 245D standards.
Engage in trauma informed care daily
Positive Support Specialist Minimum Requirements:
Minimum requirement of one year working with individuals with maladaptive behavioral history and knowledge of behavior management techniques preferred
Minimum requirement of one year of CRS, 245D, or SCS experience.
Strong interpersonal skills with the ability to work with a variety of individuals and recognize individual boundaries.
Previous completion of at least one singular training in behavioral skills/emergency use of manual restraints such as PIA, CPI, or MANDT
Knowledge of local community resources is a plus.
The successful candidate must be cleared by a Bureau of Criminal Affairs (BCA) background study and have a drivers’ license; a Motor Vehicle Check (MVR) is required.
