Position Description:

The purpose of this Direct Support Professional position (DSP) level 2 (Driver) is to work directly with people with disabilities in a community integrated setting so they can live a life filled with purpose. Rise’s Life Enrichment Programs are designed to support persons who have a disability. People in our program benefit from and enjoy participating in day activities, creating friendships, and developing new opportunities for personal growth. Our DSPs work to support individuals in our programs with customized support plans that are designed to meet their specific needs and interests. Examples of Life Enrichment Program activities include: art therapy, music therapy, cooking, YMCA classes and events, social activities (potlucks, etc), learning activities including reading and discussion groups, community activities, and special events.



This position will be based at our St. Louis Park, MN location



Responsibilities

Direct support professionals assist people with personal cares and other activities of daily living including therapist-designed programs such as walking and range of motion exercises
Document Care Plan outcomes and objectives daily.
Dispense medication per physician orders. Maintain required medication documentation as required by state licensing.
Assist Activity Coordinators in set-up of activities and encourage people during groups/activities as needed.
Continually inspire personal growth in the people we support by working on personal goals and implementing person-centered practices.
Conducts oneself as a positive role model for people and other team members in terms of appearance, socialization and teamwork.
Position does include lifting and transferring adults to/from wheelchairs
Complete documentation as needed
Provide daily transportation of persons supported in Rise vans and buses
Input information into Care Director database.
Requirements
Experience, competence and sensitivity in working with people with brain injuries and physical disabilities preferred.
High School diploma
Ability to meet the physical demands of the position and correctly use assistive and safety equipment.
Hardworking, flexible and ability to work collaboratively with a team.
Maintain calm demeanor in handling occasional difficult situations.
Previous experience is helpful, but we are willing to train the right person

Applicants must have a valid driver license and willing to drive a Rise vehicleto transport persons served

Position Description:

As a Direct Support Professional (DSP) Life Enrichment Program (ASL Proficient), you will assist with the day-to-day work and social skills of people with intellectual disabilities, developmental disabilities, mental health and who are also Deaf, DeafBlind, and/or Hard of Hearing. Fluency in American Sign Language (ASL) is required to be successful in this role. You will:

Provide job development and retention services to people served by Rise. Assist people in developing job goals, job-seeking skills, and identifying potential employers. Network and market Rise and the people we support to business partners.





Responsibilities

Provide individualized job placement services through weekly meetings with person served to work on job seeking skills, preparing resumes and cover letters and interviewing.
Actively cultivate competitive job opportunities with businesses and employers in the community.
Communicate effectively with diverse stakeholders including treatment and sober housing teams, probation officers, businesses and employers.
Equip and empower individuals to be successful in their new job opportunity through ongoing support.
Responsible for maintaining a caseload of no more than 20 people while tracking and documenting variety of data about services provided as well as billing.
Utilize person centered thinking and motivational interviewing to best engage with person served.
Requirements
Knowledge of supported employment field and one year of experience providing job placement and job development services preferred.
Associate or Bachelor’s degree in human service field or criminal justice preferred or equivalent work history of 2 years.
Knowledge and/or experience working with adults with criminal backgrounds, mental health diagnosis, chemical dependency, or homelessness.
Background in human services, social work or customer service preferred.
Ability to use electronic devices for organization (ie: electronic calendar, email, etc.).
Excellent organizational skills.

Position Summary

The Licensed Practical Nurse (LPN) will provide medical support services and organize the medical department and ensure compliance with 245D regulations. The LPN will also promote the inclusion of each person served within the community and provide training and support services to persons served. Support services include positive approaches to learning, systematic instruction, behavioral intervention, case management, and self-care interventions.

Responsibilities

Ensure medications and treatments are accurately administered according to physician’s orders and proper procedure.
Complete documentation of medications administered, treatments provided, and individual illness/ injury is completed in a timely fashion.
Ensure emergency health care needs are responded to appropriately and emergency medical care is sought as deemed necessary.
Ensure new medication orders are communicated to program staff, as necessary.
Ensure medical procedures are performed in accordance with physician orders, including gastronomy feedings.
Under the supervision of the RN, will instruct staff in proper performance of medical procedures and will monitor compliance.
Communicate clear, accurate, pertinent, and complete information regarding individuals health status/needs to staff, group homes, and to guardians according to program policies and procedures.
In consultation with RN, make decisions and arrangements for individuals whom become ill during the workday.
Carry out therapeutic aspects of programming (recommended by OT & PT) in the community and work setting.
Provide case management services to assigned caseload. Including maintaining case records, providing active treatment, and maintaining regular communication with team members.
Document Care Plan outcomes and objectives daily.
Provide direct support for the individuals in this program including lifting and transferring adults to/from wheelchairs (two-person lifts) and assisting with personal cares.?
Continually inspire personal growth in the people we support by working on personal goals and implementing person-centered practices.
Conduct oneself as a positive role model for people and other team members in terms of appearance, socialization and teamwork.
Instruct CPR/First Aid to Rise employees as needed.

Ability to meet the physical demands of the position and correctly use assistive and safety equipment including lifting 50 lbs and doing two person lifts.
Hardworking, flexible and ability to work collaboratively with a team.
Maintain calm demeanor in handling occasional difficult situations.
Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.). Case management (Care Director) – Learn software system and become proficient.
Current, valid driver’s license.
A driving record that demonstrates safe, legal driving habits.
Compliance with Rule 11 and MVR background check required.
Previous experience practicing person-centered practices with people with disabilities or other barriers to employment.
